Welcome!
This interactive Health Map gives you the unique ability to visualize the well being of the four-county Capital Region. Yet more than a map display, this site contains a relational data base that allows you to conduct your own health queries, compare data, make charts and tables, and find information on our region’s health assets. Now you can be armed with powerful information about the health of our community, right down to your local zip code. It is a work in progress, so let us know what you think! To get started, please take a few moments to view the animated tutorial below.

Why was the Health Map created?
Every three years the four not-for-profit health systems in our area conduct a Community Needs Assessment (CNA) to guide their community benefit programs (see 2007 report, left, for details). Rather than place all the information they gathered in a lengthy paper report, the health systems wanted to create a “platform” and tool to make this important health information more accessible, current and useful not just to them, but their community partners, and the general public too.

Where does the data come from?
The data used by this site came from several sources. Demographic data are based on the 2000 census but are estimates for the year 2005 made by Claritas, Inc., for individual zip codes. Youth obesity numbers are collected by individual schools and released by the California Department of Education. “Cause of death” rates come from the California Department of Public Health. Data for computation of hospitalization and emergency room visit rates come from the California Office of Statewide Health Planning and Development. The number of “Medi-Cal eligibles” come from the California Department of Healthcare Services. The national numbers come from several sources, most of which were from the Federal Government. This site uses the most recent data available for 2005 or the 12 months ending June 2006.

What is the intended use of the Health Map?
The purpose of this site and report is to help spotlight the unmet health needs in the Capital Region. This site does not create new health information, but instead presents available health information in a dynamic new format. While every effort has been made to offer the most accurate information possible, inadvertent errors may occur and some data may become outdated. Conclusions, inferences and/or judgments drawn from this information are the sole responsibility of the user and are entirely at your own risk. See disclaimer at bottom for additional details.
